When talking about telemedicine and telehealth, it is essential to differentiate between the two. According to Rubenstein and Painter, “Telemedicine is the use of synchronous audio and visual communications to deliver health care at a distance. Telehealth is the use of electronic and telecommunications technology to deliver asynchronous health care at a distance” (2020, p. 38). A Nurse Practitioner working in today’s healthcare organizations, one will find themselves impacted by telemedicine’s growth and the positive and negative implications it has when delivering non-face-to-face patient care. Furci explains that digital healthcare expansion is still making forward progress (2019). What seems to have skyrocketed this process in 2020 is the world pandemic Covid-19, which continues at present. According to Rubenstein and Painter, some of what appeared to be restricting factors, such as Medicare requirements around telecare visits and payment for services, has been relaxed to keep patients at home safe from further illness (2020). The Physician Practice Perspective Journal talks about limitations around the ability to bill for telemedicine services, patient knowledge that the services existed, and poor access for Medicare patients as challenges for growth (2017). Healthcare is fluent and changes as our populations’ age and demands on access to care change. Our current 2020 pandemic has had a massive impact on our healthcare system demands, which in turn has greatly influenced practices around telemedicine. We can see telemedicine’s benefits during the current pandemic of keeping people safe and away from potential exposure to new illnesses. Providers can also help provide telemedicine care visits to patients that are hard to reach with distance and travel concerns. “Healthcare is moving away from a fee-for-service to value-based care, which makes telemedicine a desirable alternative to in-person clinical visits (2017). There are many visits that a clinical Nurse Practitioner can have via a video call as a clinician such as support with medication management, tracking, and monitoring of lifelong illnesses that provide benefits and quality care to patients (Arnautova, 2018). This new wave of delivering healthcare virtually is here and will continue to stay as long as there is a need and a means to provide safe, quality, cost-effective care to meet patient needs.
