For this assignment, identify a program in the planning or implementation phase whose purpose is to improve an issue or problem in your organization. Create a logic model that shows the inputs/resources, activities or processes, outputs, outcomes, and impacts. For example, you might choose a new program or initiative that is included in your school improvement plan or one that is in progress. Or you might choose a new initiative or program improvement in your post-secondary institution or other organization.

Your assignment will have two parts:

Your logic model.
Your background information and data about the program or initiative, its activities or processes, and its goals (output, outcomes, and impact).
You may use the Logic Model Template provided in Resources, based on page 54 of the W. K. Kellogg Foundation guide, or you may use a different template to illustrate the relationships among the model’s components.

Provide sufficient information and data about your program for each section of the template. Use the Mytown data in Chapter 2 of the Kellogg guide. The information included should allow a reader to clearly understand the model.

The scoring guide for this assignment is based on how well the program information and data are aligned with the logic model components. This is practice.

If you have insufficient information, you may want to contact those responsible for the program’s planning and implementation. If you still find gaps in the way your program was planned and they have not been addressed by planners, identify the gaps and explain. For example, you may find an output for which no activity was planned or an outcome not connected with the model’s input elements. Some schools still develop improvement initiatives where the planning stops with the output component; the fact that an activity was conducted or completed is an output, not an outcome. An outcome would be defined in terms of the improvement expected to occur as a result of the professional development. Your explanations are important.
