This assignment follows your findings of the Windshield Assessment (I will attach below) you conducted previously, it was done Miami Rescue Mission clinic and paper should be based off that with the intervention being health education.

This APA paper should include how the community nurse would plan for change using the following topics:
Mobilize, Assess, Plan, Implement, and Track.
Utilize these sections as headers for your paper.
This will help you to learn how to begin to plan and implement a small intervention of change at your assigned community site, intervention is health Education.
1. Take your nursing intervention that was recommended in your assessment and prepare a paper.
