Goals and objectives (25 Points)
• Describes what the organization wishes to accomplish by completing this grant project.
• Identifies Measurable target(s) that must be met on the way to attaining the goal.
• Clearly identifies the goal(s) and objective(s) of the project?
• Ties directly to the objectives of the James & Jean Community Health Care Program and the Health Opportunities for Children Objectives.
• The ultimate desired outcome of this project is clearly states and fits within the business, district, school, city, etc. •
• Considers the environmental soundness and practicality of the proposal.
Work Plan (25 Points)
• Is there a clear work plan?
• Are the activities, steps or tasks (work/business plan) clearly stated and realistic
• Explains how the organization complete this grant program within the specified term of the grant.
Evaluation (20 Points)
• Will the project meet the goal(s) or objective(s) and will it provide the output intended?
• How will the organization evaluate and measure the interim progress and outcome of the project?
• Who will be responsible for measuring and reporting your interim progress and the final project evaluation?
• What is the estimated cost of your evaluation component?
Application Completeness (10 Points)
• Is the proposal clearly presented and complete?
• Provides evidence that organization has sufficient staff resources, technical expertise, and experience to successfully manage the grant project.
• Provides letters of support and/or commitment for the grant project from from entities outside the agency.
