Health Promotion Project

You have been working as a community health nurse for three years. You have identified from your experience and, from clinic data that there is a need for a health promotion project in your community for the following health issues: Ear health (e.g. otitis media, hearing loss),Mental health (e.g.depression, anxiety),Cardiovascular health (e.g. stroke, heart disease),Dementia (e.g. healthy aging, brain health, 65+ age group).
You are required to explore one of these health issues (listed above) and develop a Health Promotion Project Plan for the community you investigated in Assessment 1 (Bunbury OR Claremont). You are required to investigate the same community further using the McMurray Community Assessment Framework. You are required to choose a target group for your intervention, e.g. Indigenous children aged 0-4 in Bunbury to prevent Otitis Media; prevention of stroke in older adults in Claremont, etc.
The purpose of this assessment is to introduce you to community assessment frameworks and to explore a health issue from a Primary Health Care (PHC) and health promotion perspective. It also enables you to consider how the characteristics of the target group may influence health promotion strategies.
