As an advanced practice leader you are tasked with developing an interdisciplinary approach to a health promotion project in your facility or within your community. Ideas for health promotion activities could be: burnout prevention strategies, cardiovascular health and healthy lifestyle choices. Choose a topic based on the needs of your facility or community.

ASSIGNMENT OVERVIEW
Development of health promotion plans with patients, families, communities, and professionals is vital to the improvement of health disparities and disease prevention. As an advanced practice leader you are required to develop a plan that promotes health on the clinical unit or in your community that can be shared with clients. Select a health promotion topic and complete an interdisciplinary collaboration plan for the roll out of this project.
a. Identifies a health promotion need on the clinical unit, facility, or in the community that impacts health and includes literature support that demonstrates it’s a problem.
b. Addresses why collaboration is important and what techniques will be incorporated including how interdisciplinary strategies will be utilized. Literature support for how the plan will improve outcomes needs to be included.
c. Identifies team members for supporting this plan including how the health promotion plan will be rolled out. Literature support that demonstrates the benefits of an interdisciplinary approach to health promotion needs to be included.

ASSIGNMENT INSTRUCTIONS
2. Sources older than five years may not be used without the permission of the class professor.
3. Title page, pagination, subheadings, body of paper, citation of sources, and reference page must follow APA guidelines as found in the 6th edition of the manual.
4. Rules of grammar, spelling, word usage, punctuation, sentence and paragraph structure are followed and consistent with formal, scholarly writing as noted in the APA Manual (6th edition).
5. Please note: If you do not receive a proficient rating in any major content category, you can re-submit your assignment with revisions in those content categories to receive a better grade. You have one additional opportunity to revise after the initial submission in order to make improvements. The initial submission must be a complete paper, rough drafts will not be graded.
