Nursing Care Plan

Assignment: select one patient whom you have provided care- perform a deeper assessment (assessment form accessible in D2L)
You are able to be creative in the creation of the format of the care plan. You may use concept maps, PowerPoint, basic typed approach. Please feel free to use color. Include your sources at the bottom of the plan.
Create a Holistic Nursing Care Plan for the patient.
The Care Plan will consist of 6 Nursing diagnosis that are appropriate and applicable to the patient. Only 1 risk for diagnosis may be used. Nursing diagnosis will include the following areas;
 Physical-2
 Psychosocial/sexual/family dynamcis-1
 Medications-1
 Nutrition/health promotion-1
 Teaching-1
After creating the 6 nursing diagnosis identify 3 of them as of primary importance to your patient.
Each diagnosis will have 1 outcome, and 3 interventions (SMART) required. Interventions should address – 1 of each. (Be creative. How will the patient be engaged)
 Do (the patient or student will)
 Assess (assess and monitor are the same idea)
 Teach
Evaluate the outcome of the 3 primary diagnoses. Evaluation will include:
 did the patient achieve the outcome
 what occurred to prevent the patient from achieving the outcome
 what needs to be adjusted in the interventions to meet the outcome or does the outcome need to be adjusted
 if outcome needs to be adjusted then rewrite the outcome.
