Create a case study. Answer the following questions using the outline provided.
Give a brief description of agency and services provided. Agency is Faith Presbyterian Hospice. Make it a general one about a hospice agency.
Summarize assessment information.
Provide a summary of the client’s demographic description and any pertinent information about the family system. Demographics consist of ethnicity, race, education, knowledge, age, marital status, gender, and social economic status.
What is the presenting problem? Provide a human behavior and social environment (HBSE) theory to explain how and why you developed your conclusion.
Explain the effect of socioeconomic status on client. Support your argument with research regarding the effects of socioeconomic status on human development, education, and/or health.
Define culture and its significance in social work. Based on the definition of culture, define and explain a minimum of two cultural factors that affect who client is or his/her situation.
Explain how your client’s culture and ethnicity impacts the presenting problem.

Who else is affected by the problem? How are they affected by the problem? Apply systems theory and include answers in the micro, mezzo, and macro levels.

1. What do you mean by create a case study?
I’ve learned that the inclusion of fictional information is necessary for some papers because if the case study is all factual, then I would receive answers, “not applicable,” because the lack of opportunity at their field placement. Creativity has been necessary to complete the paper with thorough answers.
Thus, the paper may have both real facts and fictional information and will depend on your field experience overall.
IF working in groups, the first step of the assignment is to talk to your teammate about your agency, its services, and particular experiences at your respective agencies. For all, the next step is to “create” a client based on your experiences and select a setting (your agency). “Create” herein means to use real facts in connection with fiction where necessary.
Overall, the goal of the paper is to answer all questions thoroughly to present your understanding of the helping process.

3. How long does the paper need to be?
Length is determined by articulation and detailed information. If you were to provide the paper to another social worker, would s/he be able to understand the client and the aspects of services and information provided to the client? Could that social worker continue to work with that client based on your information? Each answer should be thorough, clear, and concise.

4. Use of abbreviations and acronyms:
Be sure to spell words out rather than use abbreviations or acronyms. If using acronyms, the proper format is to spell it out when first using it on the paper. E.g. cognitive behavioral therapy (CBT); then use CBT for the rest of the paper.
