Intervention Plan (K-3)
In the Module 2 Application, you will conduct informal reading assessments
with two K-3 students. You will use a variety of assessment tools to identify performance levels
and instructional choices to increase student achievement. Strategies will be selected from
research-based practices.
Course Objectives
• Describe a decision-making system’s approach to intervention.
• Describe intervention strategies correlated to introduce, develop, and enhance reading
skills and abilities across developmental and grade spectrums.
Directions:
1) Create a Word or text document for your response. Use 12-point Arial or Times New Roman
font. Follow APA (6th edition) format.
2) Create a title page and references page in APA (6th edition) format.
3) Follow the directions to submit your final Word or text document.
Step 1. Select two students between kindergarten and third grade to complete two diagnostic
reports. While it is optimal to select students at local schools, you may choose your own
children; children attending summer school, a community center, or other community
organization, or the children of neighbors or friends. If needed, check with the organization to
complete any authorization forms required when the children are not your own.
Step 2. For each report, use the intervention plan (IP) template to convey your findings. Provide
specific details, including observations. Report information using grade levels or percentiles.
Step 3. Identify three target areas.
Step 4. Using a range of researched options, select two potential instructional activities
specifically for each target area. Following the directions on the template, briefly describe how to
use the strategy, connecting it to research.
Step 5. In the Comments section, address the decision-making process you used to reach the
target areas.
Step 6. Submit each report, using appropriate APA citations and references, including any
additional documentation to the assignment link.
Step 7. Save the document to your computer for possible use in Module 5.
