Preparing a Diagnostic Report
In this Analysis assignment, you will use a modeled intervention plan format to prepare a
diagnostic report based upon a case study. You may access the case study and the modeled
intervention plan template from the assignment page.
Course Objective
• Analyze data to determine targeted needs within components of the reading process,
including phonological awareness, phonics, fluency, vocabulary, and comprehension.
1) Create a Word or text document for your response. Use 12-point Arial or Times New Roman
font. Follow APA (6th edition) format.
2) Create a title page and references page in APA (6th edition) format.
3) Follow the directions to submit your final Word or text document.
Part 1: Diagnostic Report
Step 1. Access the case study from the assignment page. Read the case study provided,
identifying areas of potential concerns which indicate how this reader is struggling. Gather
background data and other observations given within the case study description.
Step 2. Access the intervention plan (IP) template from the assignment page. Use the template
to report your findings. Provide specific details, including observations. Report information using
grade levels or percentiles.
Step 3. Identify three target areas.
Step 4. Using a range of researched options, select two potential instructional
activities specifically for each target area. Following the directions on the
template, briefly describe how to use the strategy, connecting it to research.
Step 5. Submit your report, using appropriate APA citations and references,
including any additional documentation to the assignment link.
Part 2: Prepare Assessment Instruments
Step 1. Using the materials in a published IRI, copy, mount, and place student documents in
plastic protective sleeves or laminate. This includes one set of word list forms for all lists. It also
includes one set of forms of passages.
Step 2. Make five copies of the assessment documents for the IRI, including the word lists and
passage pages for teacher notes, and the summary pages for the summative report.
Step 3. Prepare any other materials used for the informal assessments as described in the
presentation such as the chunking exercise or problem- solving puzzles.
Step 4. Assemble all the materials in an easy-carry tote along with paper and pencils.
Step 5. Write a two-paragraph reflection on your approach to preparing the assessment
instruments. Use the following questions to guide your reflection:
• What was your thought process as you located, prepared and assembled all materials?
• What challenges did you encounter?
• What other materials could be included?
