Substance abuse disorder

The next step in the Community Health and Wellness Promotion Plan to assess the best practices for evidence-based interventions for the primary health concern, and to identify community resources that address this concern. You will submit a 3-4-page APA-formatted paper that basically investigate answers to three questions:
• What works to address this primary health concern and population?
o Describe three best practices (programs or policies) from a variety of sources, including scholarly resources (including public health organizations and websites listed below):
• What programs, services, supports, and partnerships are in place to address this concern in your community?
o Describe 4-5 community programs, organizations, or partnerships (coalitions) that address this concern in your community. Include program name, description, activities, and how they measure success.
o Conduct personal interviews (required), visit community organizations, community forums or coalition meetings; visit websites, social media sites, and/or written materials
• What gaps or unmet needs are evident from the perspective of the community?
o This may be an aggregate from your interviews with members of the community, providers, partners, and stakeholders.
